My research is on critical illness insurance/ compensation among insured. So survey questions for factor influence the uptake or awareness would be best. So far I have go through literature review, most studies are on medical insurance ,health insurance or life insurance.

Some areas of study have banks of validated questions. These typically are in well-researched topics such as tobacco. It sounds like your you may be breaking new ground. However, you may be able to modify items that have been used in similar areas. See if you can find validated surveys that ask about uptake or awareness of insurance/compensation of other types such as cancer insurance. Then modify the item to focus on critical illness.
