A Dor-fundoplication accomplished after anterior extramucosal myotomy (this is not exactly Heller !) is easier to perform (unlike Toupet) and has the advantage to protect the exposed oesophageal mucosa. The Dor-fundoplication helps to prevent an unacceptably secondary gastroesophageal reflux. Having in mind that children are growing up and the esophageal transitional zone is changed through the years towards presumed shortening of the esophagus, I prefer to fix with three stitches the fundus to the diaphragm. For a period of 20 years I had no the chance to meet consequences with incidence of dysphagia in the long-term.
