A male patient of 70 years has his random blood glucose 78 mg/dl, serum creatinine 7.8 mg/l, serum urea 101 mg/dL, nil urine protein and sugar, hemoglobin 13.3 gm/dl? What is the probable diagnosis?
The urea;creatinine ratio is 12.9 (i.e. less than 15), so the patient most likely has renal or post renal azotaemia. Possible diagnoses include renal failure, or obstructions to urine flow (cancer, uroliths or an enlarged prostate). Additional tests including measured urine production, urine specific gravity, serum phosphate and electrolytes would be helpful. Abdominal ultrasound should also be considered.
