We currently use a weight of 1800 grams, greater than 34 weeks corrected gestation and an air temp of 27-28 degrees to consider placement to open crib.
What a good question and I would like to learn from everybody. Our group covers mutiple units and different units use different criteria. Most of the time it is up to the nursing staff. Some nurse will not even take the premies out unless they are full PO. I have read 28-28.5C as the temperature to come out in one book and I use it. I always encourage the parents or nurse to give it a try if they feel the baby is stable enough but it is just me. The smallest one I sent home was 1450 grams severe IUGR, full PO and gaining appropriate weight, out of isolette for 4 days, and with very caring parents.
Our unit was involved in a NICHD study that examined the early weaning of infants from isolettes. We dressed and swaddles some of these infants per the study guidelines. The results may well be available as its been about two years. Best of luck to you!
While the weight of the preterm is an important consideration, the calculated GA of the baby is very crucial. It would be disadvantageous transfering preterm at less than 34 weeks GA even at1.5 kg or more because of risk of feeds aspiration accompaning poor coordination of breathing with swallowing at less than 34 weeks.
