Different field of sciences have still remaining open questions. Medicinal chemistry is a frontier science, involving diverse other expertises. So, it is easy to find the hot topics in this area. However, its limitations seem to be a few neglected, mainly those associated with fundamental concepts. So, what are the most relevant open questions in medicinal chemistry field?
The extent of the unsolved areas in medicinal chemistry can be variable to different researchers in their own ways.
However, I think the recognition of the appropriate ligand by the target or the vice versa, is still one of the open questions.
I mean, the activity of the molecules can be explained by the interactions, pocket size etc. but how they get specifically recognized, intrigues me more.

While they are being treated with drugs, i.e. molecules that have a certain mass, chemical structure. Depending on the temperature of the body (36-37 0C), they undergo structural changes and have a certain thermal energy kT. The same applies to vaccines.
But each drug molecule and the molecule of human body has its own minimum energy or zero point energy. It affects the movement of a proton in the water of the human body, the exchange of information in the human body at the quantum level. So far, this area of human health science has not been developed. Physicists who can develop this science are engaged in quantum communication, quantum computers, photon teleportation...The direction of the science of quantum human health is waiting for its researchers.
