What are design criterion to safeguard iron and steelmaking furnace from steam/gas explosion?

Suppose lance leak in BOF, cooling water lining leak in induction furnace/EAF roof, or unsorted scrap containing perfume/aerosol cans/gas cylinders charged into furnace. Practices are paramountly important, but what are design parameters of furnace to prevent steam explosions?
