Clinically, how do you differentiate TB colitis and Crohn's disease, especialy if you fail get a definite tissue diagnosis? Radiology?
This could be challenging for those who practice medicine in endemic area of TB.

Acid-fast stain could tell the presence of mycolic acid of Mycobacteria. (PS: I am not a clinician)

TB PCR on tissue,
histology if adequate tissue biopsy or resection is different
Crohn's has an inflammatory component much higher than TB
Crohn s granulomas are hardly ever necrotizing
The special stains are not sensitive enough to detect TB in too many cases

Yup, we do found that acid fast bacilli may not sensitive enough to detect TB.
