Hi Najmul,

There is no such thing as a structured qualitative questionnaire. That is purely the domain of quantitative research.

You can only have unstructured, and more commonly semi-structured, interviews in qualitative research.

There is some confusion in your last reply. There are different uses/types of questions within the research process. For example, researchers (whether they are qualitative or quantitative in their approach) use research questions, aims and/or objectives to state the original purposes of the study.

There is also the use of questions within the data collection stage - such as questions used during qualitative interviews which are typically semi-structured or open questions. Alternatively, questions can be closed/structured, typically used in quantitative research (as Dean mentions too). 

Some background reading of introductory research texts should provide useful additional information.
