Any idea what the author try to imply between the choice "Never" and "No chance at all" for SBQ-R under item 4?
How likely is it that you will attempt suicide someday? (Circle only one):
0 = Never
1 = No chance at all
2 = Rather Unlikely
3 = Unlikely
4 = Likely
5 = Rather Likely
6 = Very Likely
Any response on the SBQ-R that is scored a zero (0) applies to only individuals who have Never, in their LIFETIME, considered seriously or engaged in suicide-related behaviors.
A score of "1" relates to those with PAST ideation but low likelihood of ever doing so again...
It is unfortunate that people simply post these types of instruments without asking our research team for the "manual" and how BEST to interpret the scores on this "clinically-derived" instrument (see APA guidelines on the selection and use of instruments). Our team did NOT give permission for the public posting of the SBQ-R...

Bob, the developer has given insight on the issue you raised. It always good to check the manual and/or ask from the original author.
Many thanks Prof. Osman and Dr. Mansor. I strongly agree that consulting the author and the "manual" are the best ways to understand the difference between the two answers to item (iv).
However, I have a question from a colleague who mentioned that whether it is good for her to explain this interpretation of the two different wordings to her class before the survey so that the respondents understand too? Initially, she has thought the questionnaire is fully a "brief self-report measure" and the instruction read: "Please circle the number beside the statement or phrase that best applies to you." She is wondering that if we who are researchers in this line are also confused with the difference between the two wordings, then what about the university students who are answering the questionnaire based [wordings] best apply to them?
I think I can understand the interpretation and the thoughts behind it ... many thanks to Prof. Osman's clinical explanation. But now, I think the question becomes whether the people who are administering the survey such as those in university classrooms should elaborate more "verbally" or "in writing" specifically for item (iv) instead of just relying on the standard "instruction" on the questionnaire, so that respondents can understand them correctly before answering.
