I would insist on a full expose on the rules and regulations that Ethical Committees of premiere MEDICAL INSTITUTIONS follow before sanctioning randomized controlled trials. (e.g., MIST TRIAL). Equally insistent would be my emphasis on EDITORS who accept such articles for review, the choice of reviewers, the hiding of FINANCIAL CONTRIBUTORS far away as possible from the MAIN PAGE, as it done by NEUROLOGY.
The FDA, like the CIA and the Mossad or the KGB is entirely beyond control, so it seems futile to even try to rein them in. But, an effort in the direction of the FDA and Science affects all humanity; the others can expend a few agents here and there, always. John Lennon's IMAGINE is working all right -- in REVERSE.
For a long period of time, around a decade or so, NEUROLOGY (AAN) refused to accept correspondence from non-AAN members of the medical fraternity.
I would insist that all migraine patients be treated with propranolol + amitriptyline in maximally tolerated doses for at least six months before being recruited into a RCT, and those that cannot be so managed be excluded from RCTs. I predict that over 95% of so-called "refractory migraineurs" inducted otherwise with such ease in RCTs will remain well for decades on this combinations. Weight-conscious persons can always increase their cigarette-smoking by a pack of 20 or more (see my article on nicotine for migraine-prevention), in the process giving our cardiac and oncology colleagues a bit more work in exchange.
I would like to put the Industry on trial for pushing RCT in all possible medical fields without a proper theoretical matrix. Now, proper is a word that is ALWAYS debatable. I underscore that wherever more than one human brain works or a number of human brains work without complete cohesion and self-correction and overriding of their own over-enthusiasm and belief-in-belief religion-like rigidity and self-righteousness, politics enters the sphere and the cause of the science is LOST INDEFINITELY.

I believe that this QUESTION would be my greatest contribution to SCIENCE FOREVER.
