Logically, a migrated wrap is due to a failed crural repair. So why not simply reduce the migrated wrap, mobilize adequate length of esophagus into the abdomen and close the crura securely?

migrated wrap is not due to a failed crural repair, but due to incomplete mobilisation of the lower end of the esophagus. Before embarking on crural repair, one has to ensure atleast 4-5 cm of esophagus remains in the abdomen at rest. for migrated wrap, its important that the length of stomach be mobilised, crural repair or reinforcement and that the redo wrap.
