A 50 years old lady had RTA followed by multiple rib fractures (right) with huge hemothorax (right), huge hemo-peritoneum due to liver and kidney trauma. I inserted a chest drain, DPL (diagnostic peritoneal lavage) through which more than 1 L (both) blood came out. No evidence of gut injury noted by DPL. Do I need laparotomy for this patient?
Kidney trauma is subcapsular. Liver has healed (stop active bleeding) spontaneously revealed by DPL drain. @ Natasa Viskovic Filipcic
Natasa@ Yes Mam, she is doing fine. Her bowel movement is ok, abdomen is soft, hemodynamically stable, no respiratory problem. Follow up CXR and USG abdomen is fine. Can I now discharge her? (I have removed chest drain and DPL drain.
I posted the question 7 days back. At that time she was in ICU/HDU. When her hemodynamic status became stable (5 days back), I transferred her to ward. I removed chest drain and DPL drain 1 day back. And the follow up CXR and USG has been done today. @Natasa
There was no history of hematuria at any time and the subcapsular hematoma is reducing. No remarkable hematoma (on USG) noted in Liver. At present, no discharge by the DPL wound. Thanks for your compliments..... @Natasa
