Hematogenous infections can produce late prosthesis infections in both orthopedic and urologic prostheses.Antibiotic prophylaxis for minor surgical procedures and dental procedures remains controversial, however. Carson and Robertson reported a series of late infections from remote entry sites including dental abscesses. They did not recommend routine prophylaxis prior to dental procedures, as it does not appear to be cost-effective.

Jacobson et al created a decision analysis model for antibiotic prophylaxis in dental patients with joint prostheses. Their conclusion was that predental antibiotic prophylaxis for patients with prosthetic joints was expensive, but not cost effective. 

In highly selected patients, however, especially those with immune compromise or dental abscesses, prophylactic antibiotic coverage may be appropriate.

In patients with dental infection the antibiotic prophylaxis of hematogenous infecting of hip or knee prostheses is mandatory. Late deep periprosthetic infection has usually a dental or urinary tract origin.

Antibiotics are required for patients with dental infections, and good oral hygiene.

Otherwise antibiotics are not recommended for routine dental hygiene procedures.

There are guidelines from the American Academy of Oral Medicine, the American Dental Association, the American Academy of Orthopedic Surgery, and the British Society for Antimicrobial Chemotherapy regarding patients with prosthetic joints.

It is not mandatory antibiotic prophylaxis in urologic prostheses for dental surgery.

On rare occasions dental bacteria may infect artificial joints or valves.  However, does this bacteria originate from one-time-only dental treatments?  Or does it originate from chronic dental infections/inflammation that allow oral bacteria access to the bloodstream after every toothbrushing, or even 24 hours/day exposure to oral bacteria if a patient has chronic gingival inflammation that leads to chronic gingival bleeding?  There is no way to determine if joint infections involving dental bacteria originate from one-time-only dental treatments or from chronic wounds/inflammation in the mouth.  This inability to specify this causality compromises the scientific rationale of antibiotic prophylaxis. 

Also, will antibiotic prophylaxis result in antibiotics reaching the blood stream at a therapeutic concentration at precisely the moment when a one-time dental procedure puts dental bacteria in the bloodstream?  There is no way to ensure that timing.  Also, how does one know that the antibiotic used will be effective against the dental bacteria in the blood stream, versus the bacteria being resistant to that antibiotic? There is no way to tell.  The science supporting antibiotic prophylaxis is weak at best. 

How did the original inventors of the concept of antibiotic prophylaxis in dentistry address these questions?
