Optic Nerve Sheath Diameter measurements are of maximum interest in the management of patients with hydrocephalus and other types of intracranial high pressure states. Probably comparative of optic nerves and nerve sheath relation in different clinical situation could be prognostic.
I think serial measurents can be very useful in follow up of shunted patients in hydrocephalus. On one side, it can ensure among other things that the shunt is draining no more and no less. Neurosurgical researchers have also found that watching retinal veins in fundus examination could detect shunt overdrainage. You might say that ophalmologists might have other ways of testing optic nerve function. But suppose for some particular patient there is some irreversible optic nerve damage with some visual loss, optic nerve diameter ensures shunt is offering best condition to keep his remaining optic nerve function.
On the other hand it can be used as an indicator among others to ensure optimal valve adjustment which is a key factor of hydrocephalu recovery.
But to note that in some hydrocephalic patients optic nerve and vision is not affected. Others can tell you what can be the potential value in such case.
