To my knowledge, there is no link between diabetic neuropathy and central pain syndrome. Any patient with diabetes mellitus may, however, develop the central pain syndrome.
Essentially diabetic PNP is a peripheral neuropathy, although there are some arguments for lesions of the sensory neurons upstream from the dorsal root ganglion, which is principally affected. Thus, it is classified as peripheral neuropathic pain, contrasting with central pain where there is no lesion outside the CNS explaining the pain sufficiently.
