I have experience in molecular biology, specifically cloning, expression and purification of bacterial proteins, but my interests are shifting towards stem cells, Immunology and autologous transplantation. Would I know about possibilities to switch or accommodate my current skills with my developing interests?

there is probably no standard recipe, of course. I'd find some interesting literature about the desired new topic and then try to identify problems/questions I could address with my current method repertoire and those methods I'm interested in to acquire. Normally it's a process that takes some time but at some time you should have a sudden "Heureka!" experience.
If you can find a group specialized in your subject of desire, get in touch with them and find out what questions they are trying to answer and how you could contribute to their success.

I find your question rather puzzling. If you have experience in molecular biology, you should be able to apply that knowledge in your new fields of interest quite easily and effectively especially in Immunology. I am sure you are aware that there are lots of molecular and genetic problems in a number of immunological conditions- perfect research opportunity. Just to let you know, my day time job is Clinical Haematology but my research interests have been molecular biology, immunology and now it is nanotechnology. Just because you have certain kind of experience, you shouldn't be constrained in your academic interests. Think BIG and BROAD. Good luck
