@Sourav I don't think there is the need of taking prior permission for using a theory in your study. Proper citation would be sufficient. Thousands of published studies have used different theories and models viz. Theory of Planned Behaviour, Technology Acceptance Model, Effectuation Theory, Theory of Human Capital, Job Demands and Resources Model, etc. and the authors have only cited the proper references.

I guess this varies between different universities and countries. It used to be, and I quote: “Historically, images were reproduced in dissertations and theses without obtaining permissions from the copyright holders. Because of the clearly academic, non-commercial nature of theses and dissertations, and because access to theses and dissertations was typically confined to an academic, library setting, there seemed to be little dispute that the incorporation of such images into these or dissertations was a fair use.” (https://libguides.unm.edu/images/theses)
I think this still holds in various occasions. However, and I quote again: “As theses and dissertations began to be posted to online repositories, the publishers of those repositories sometime required that graduate students posting theses to their repositories obtain copyright permissions for images.” (https://libguides.unm.edu/images/theses)

Your last question is clearly no. Authors do not have to give permission (if applicable the publisher who did publish their work might). In case of tables for example (which you retype and edit etc.) a proper citation will do. See for example: Marsh, D. (2008). Protein modulation of lipids, and vice-versa, in membranes.
