How did you call your SNPs? Most pipelines create a VCF file with all the information regarding the quality and details of each SNP.

For consistency I would use HGVS nomenclature for all your alleles:

the description is a little difficult to follow, but simply if you have an A to G SNP at position in 203 in the gene it becomes: c.203A>G

If you do have a VCF file, I'd recommend you use VEP to give a more human understandable descriptipn of your calls:  http://www.ensembl.org/Homo_sapiens/Tools/VEP?db=core
