I would like o design a research aimed to detect specific immonhistochemistry biomarker. This biomaker is linked to the advancing grade of the disease. I want to design a case - control study. For the cases; is it better to select just random cases or should I select equal number of each grade of this cancer?

Ideally, you should select different grades and then see the presence or expression of the bio-marker with the advancing grade. You can assign equal number of patients based on the grades of the disease and then see the protein expresssion of marker in IHC in comparison to control samples. Looking at different grades and not doing it random will also help you to delineate the association of that marker with severity of the disease. So it's better to select patients of different grades and classify them in groups.
