In the recent past agents like cpp-app , bio active glass, xylitol and other agents have shown to effectively counter initial carious lesions in tooth enamel, there have been many invitro studies supporting the same, though long term animal or invivo studies are required , never the less these agents have shown promising results to replace fluorides as the major agent for countering tooth caries.
All of the above, including Fluoride. is the answer I would give in my practice both in caries reduction and in periodontal maintanance. Periodontal care including remineralization of root surfaces after scaling and root planing is a concept that is not fully appericiated. Smooth hard surfaces end up being easier to clean both with home care and when the patient receives a periodontal therapy in a clinical setting. The difficult part is establishing the appropriate protocol and consistantly applying them.
Certainly, but the larger question is how are the newer materials responding when compared to the age old era of fluorides, which according to recent report are causing higher cases of fluorosis specially amongst children. Also these materials have proven to remineralize as well as desensitize the tooth surface as effectively, if not less, when compared to flourides..
My group published 2 articles ( 3d one is coming) about new ways of supplementation by CPP-ACFP both on normal and MIH enamel in vivo. The product embodies the updating of supplements where calcium is low and/or hypersensitivity high.

As we all know enough evidence ,is present on cpp-acp . can be found in the literature regarding its effectiveniess compared to fluoride containing dentifrices. but how effective and promising are the the more advanced/ recently introduced materials like hydroxyapatite and organic fluorides (amide fluorides) , bioactive glass (novamin technology) are when compared to CPP-ACP & fluorides .
Our group has done a study regarding the same and the result have been promising.
