How do you "practically" qualify a mechanical component (gear/crane boom) or a member in a steel bridge to be safe against fatigue? Just the SN curve?

In case of bridge members I understand that the connection detail is taken into account. What about other components? Do you use the standard SN curve but with the hot spot stress? In such case which Euro code do you adopt? And does one "practically" study crack growth life in such large members?
