How do we determine misconceptions in the student's knowledge?

Misconceptions in student's knowledge in basic biomedical sciences such as physiology, biochemistry, pathology, pharmacology, and anatomy could affect their understanding of clinical medicine and their ability to apply knowledge.

How can we identify misconceptions in the student's knowledge? What are the approaches do you have in your school to determine student's misconceptions? What actions/approaches do you have in the curriculum/assessment system in this regards?
