I want to measure resitivity of circular disk, diameter is 1 cm and thickness is 0.5 mm. on basis of resistivity, i have to measure conductivity in four probe system. probr distance is 0.2 cm.
Kindly, suggest me formula of measurements and reference related to it which consisting proper formula.

You need to use a specially made conductivity meter for such a sample for reliability. However, if you want to measure it by yourself, you may design and make a four-pin device to do so. Basically, as long as the distance between the neighbouring pinsis much larger than the thickness, and the sample is uniform, the measurement could give a reasonable result. There are some publications on this method, but you should spend your own time to search for them.
You should be able to get the resistivity from a measurement of the DC resistance across the sample and it's geometry. Which direction you take the resistance measurement depends on the specifics of your samples shape. "circular shape" is a little vague.
