HI: This is Leslie Montgomery of San Jose, CA, USA. We measured the fluid distribution of both Saline and Hespan during simulated hemorrhage and resuscitation of pigs. My question have you tried the infusion of Hespan in response to hypotension.? We found that Saline goes almost immediately into the interstitial space while Hespan remains in thevascular system for a much longer period of time.
Les: there was no Hespan infusion. The goal was to compare used infusions. Best, M>

it is known, that NaCl has a very short half life (4 - 8 min) in the circulation. So it's clear. that you have a short time effect. For therapeutic issues, socalled plasma expanders (hydroxyethly starch, albumin etc.) is much more effective to increase the intravasal volume and act therefore against hypovolemic problems.
There are two simple methods to get more inisights:
1. We measured the dilution of plasma proteins (IgG, IgM [which is mostly present in the intravasal volume only], Albumin and total protein. So you can calculate if you know the blood volume and the hematokrit the plasma volume (intravasal) so that you can estimate the volume which enters the interstitial space.
2. Impedance spectroscopy (using a "body composition monitor" http://bcm-fresenius.com ). This is a useful and simple method to control volumes flows.
We use both methods to get more insights into plasmapheresis. See attached files.
