Aural cholesteatomas (CA) usually occur as late complications of chronic otitis media. They are usually unilateral, a point not addressed by conventional theories of CA formation.

If indeed they are a result of infection, they would be expected to occur bilaterally and simultaneously. However, I have not so far found such case series or even individual cases which can be reviewed and critically assessed. So please can I have references to such cases?

Note: I do not want bilateral congenital cases, ie no ear infection involved; nor bilateral external auditory meatal CAs; nor acquired CA with ear disease where the CAs are unilateral, or bilateral and sequential or bilateral and alternating; nor where CAs are part of some other systemic disease or connected with congenital malformation syndromes.
