Often the pain threshold is lowered when people go through an emotional conflict that causes fear and anxiety. In this situation, very slight harmful stimuli that in normal state of emotional stability would cause only discomfort or pruritus, in anxious people, the sensation and perception would be of pain.

Con frecuencia el umbral del dolor disminuye cuando las personas pasan por un conflicto emocional que provoca temor y ansiedad. Ante esta situación, estímulos nocivos muy leves que en estado normal de estabilidad emocional ocasionarían sólo incomodidad o prurito, en personas ansiosas, la sensación y percepción sería de dolor.

El antecedente anterior explica cómo en personas que se someten a intervenciones quirúrgicas, es fundamental identificar previo a la cirugía, el nivel de su estado emocional, a fin de que la medicación pre-anestésica disminuya el nerviosismo; ya que si éste persiste, el postoperatorio inmediato será tórpido con dolor difícil de controlar con los analgésicos convencionales.

Certainly - just look at those poor souls who have lost the sense of pain such as those suffering from leprosy.  They suffer from lesions and lost fingers that have been burnt or severely damaged becaus of loss of the withdrawal reflex which is initiated by pain.

Indeed, the case of patients with advanced leprosy of their neuropathy is very illustrative of what happens with the loss of tissues and part of external organs such as the nose and fingers and toes. In them, certainly, the withdrawal reflex does not work against noxious stimuli, because neuropathy no longer feels the pain.

Efectivamente, el caso de los pacientes con lepra en estado avanzado de su neuropatía, es muy ilustrativo de lo que sucede con la pérdida de tejidos y parte de órganos externos, como la nariz y los dedos de manos y pies. En ellos ciertamente ya no funciona el reflejo de retirada ante estímulos nocivos, debido que por la neuropatía, ya no sienten el dolor.

There is a very rare and congenital condition, which is characterized by insensitivity to pain from birth, and is also associated with anhidrosis (lack of sweating) and mental retardation.

Suspicion of this lack of sensitivity to pain is identified by the parents of affected children, when they are burned or seriously injured by accidents at home. Children do not realize the damage they are suffering from not feeling pain, and therefore, they do not withdraw from the toxic or harmful stimulus. They can even die without pain.

This is an extreme example of how pain is indispensable for the survival of the human being.

Existe un padecimiento muy raro y congénito, que se caracteriza por insensibilidad al dolor desde el nacimiento, y que además se asocia con anhidrosis (falta de sudoración) y retraso mental.  

La sospecha de esta falta de sensibilidad al dolor la identifican los padres de los niños afectados, cuando estos sufren quemaduras o traumatismos graves por accidentes en el hogar. Los niños no se dan cuenta del daño que están sufriendo por no sentir dolor, y por lo mismo, no se retiran del tóxico o estímulo nocivo. Incluso, pueden morir sin dolor. 

Este es un ejemplo extremo de cómo el dolor, es indispensable para la supervivencia del ser humano.

As I mentioned in the previous case, the congenital or birth failure of pain leads to many complications, including irreversible damage to the body due to lack of pain, as a means of protection to avoid damage or decrease it.

Also the absence of pain is known as anodyne, which may be a result of a non-congenital disorder but acquired by trauma, infection, ischemia or neoplasia in the brain, spinal cord or peripheral nerves.

On the other hand, this term has other meanings; Such as the absence of pain in a portion of the organism or an extremity. Case of gangrene that destroys nerve endings in a limb, or quadriplegia and paraplegia by high spine (neck) or low (lumbar) section.

Como menciono en el caso anterior, la falta congénita o de nacimiento del dolor, lleva a muchas complicaciones, entre ello a daños irreversibles en el organismo por falta de dolor, como medio de protección para evitar daño o disminuirlo.

También la ausencia de dolor se conoce como Anodinia, que puede ser resultado de un trastorno no congénito sino adquirido por traumatismo, infección, isquemia o neoplasia en Cerebro, médula espinal o nervios periféricos.

Por otro lado, este término tiene otros significados; como por ejemplo, la ausencia de dolor en una porción del organismo o una extremidad. Caso de la gangrena que destruye terminaciones nerviosas en una extremidad, o cuadriplejia y paraplejia por sección medular alta (cuello) o baja (lumbar).
