the idea of treating LTB is biologically sound only in very low endemy settings, where a person with a raectivation of its LTB could spread TB to its unprotected contacts. True it is that such treatment is fraught with difficulties, firstly because the agent cannot be isolated and studied in vitro -- by definition.
In high endemy settings the persons who have encountered MTB and have recovered, perhaps without clearing the infection, the majority, LTB by definition, have a solid immunity against MTB, which is reinforced by repeated encounters with the agent. Their probability of getting TB by reactivation of their LTB is lower of the probability in a person without LTB. The Koch phenomenon, and its interpretation, still holds.
Control of TB in high endemy countries must follow the course that was successfull elsewhere: better living conditions, effective detection, treatment and isolation of new cases...
The latent and active Tuberculosis have same susceptibility to antimicrobial agents that depends on the susceptibility of infecting mycobacteria . The difference that latent Tuberculosis is treated according to certain criteria while the active one is always required treatment by antimicrobial agents.
