In premenopausal women with IHD, there's a strong possibility of endothelial dysfunction as the pathophysiology beyond or even in the absence of significant cholesterol plaque buildup. So, yes there is a different pathophysiology at play in most many instances when comparing ischemic heart disease in males and premenopausal females.
Women (premenopausal) have less incidence of CAD as compared to men and this is primarily due to oestrogen. But recent data suggest that effect of risk factors on CAD differs between male and female. Effect of obesity, smoking, diabetes and depression is more in females than male. Inactivity and discrimination is also more in females than males. Thats why prevalence of CAD is more in these conditions in females (postmenopausal) as compared to men.
