This is the only national database of high school injuries in the US and has been in operation since 2005/2006.

In Sweden more than 80% of the sports federation have their mandantory injury insurance for all athletes in the same insurance company. I have validated the data from this database and are now using it for research on acute sport injuries in different sports at a national level in Sweden.

We are doing some research on prevalence of injuries among players Chhattisgarh, India.

The Swiss data  as recommended by Jiri Ambros  - - Switzerland: http://www.bfu.ch/de/forschung-und-statistik/statistik (selection Sport und Bewegung)  are highly recommendable since accident insurance is practically  compulsory and the insurance companies are empowered to enforce prevention, high quality treatment and to control outcome. Please note that the results under www.bfu.ch are available in French and English.

there are an IOC recognized centres researching in sports injuries. E.g., The Australian Federation University settled in Ballarat (Victoria, Australia) has a specific institute with several database.

The NCAA collects data I believe yearly for collegiate sports.  It measures injuries across all NCAA sports, across disciplines, and according to exposure, including practices and game situations.  Not sure if this of interest, but thought I would post.

If you are looking at sport related fatalities the National Center for Catastrophic Sports Injury Research is a great place to look. The center is working in conjunction with the University of Washington and the University of Connecticut to investigate all causes of death during sport.
