what're the suitable methods and tests for the detection of a significant titer of antibody (cut-off) for a disease in order to say that this patient is considered (infected cases that require treatments) or just has a titer of Ab due to previous exposure to the pathogen without a significant Ab titer and not need to treat.

I think you could check the kits manual and also could consider a regression and ROC curve during statistics to detect a cut off value.

There is no universal antibody titer you can use as cut-off. Usually for common pathogens in order to be sure that particular pathogens is a causative agent of the disease, you can determine the titer at the onset of the disease and four weeks later and compare both antibody titers. If the titer increase is at least 4- fold you have the etiological factor (but your patient has probably already recovered). The patient's specific antibody titre can also be compared with specific antibodies in the reference range (from manufacture kit or healthy people) increased by 3 standard deviations. In other words you may calculate the mean of normative values + 3SD.
On the other hand, for some kind of "unique " or not so common viral pathogens such as HIV or HCV any presence of specific antiviral antibodies suggests infection and an indication for treatment.
Detection of pathogen specific IgM indicates recent infection.
