In the latest publication of The Lancet Global Health, I noticed that still antimicrobial resistance remains as the biggest challenge of this planet to fight against infectious diseases. Why the global community fails to address the challenge despite different interventions around the Globe?
Many projects have been started In the last yars to solve this problem. unfortunatly the Use of antibiotics for alymentary industri as well as the inefficient prescription in hospitals still remains one of the most difficult problem to solve.
The most important liming factor is the time needed to have a positive antibiogram and consequently the possibility to prescribe the right antibiotic to the patient. In the most of the case an empiric treatment is needed before the result of the bacteria culture. In other case physicians used large spectr antibiotics for all infection causing un increasing of the general resistance.
For this reason many technical trials are ongoing to find an alternative to classical culture and allow a more focused and appropriate use of antibiotics (And general drugs use).

It is a very tough challenge to prevent development of Anti Microbial Resistance (AMR) because of the presence of multiple independent factors and its interaction among them.
In most of the situations the causes are either not known or have not yet addressed. At a glance people blame it to the doctors, and to some extent to the patient and its party for giving pressure to write down antibiotic for speedy recovery.
Again the problems are different in different countries and again in the same country AMR problem varies from region to region. In our recently concluded study in a tea garden area we found out multiple issues, that we did not know earlier like role of management, presence of alternative medicine man (sells medicine directly to the patient), presence of laboratory facility etc. etc. etc. (list is lengthy).
So whenever we think about policy, it has to be introduced in presence of all the stakeholders involved with the decision making.
So the problems are not uniform through out the world and it has to be customized from region to region. Thank you.

To fight Antimicrobial Resistance (AMR) the new drugs development is very important, but not sufficient. Based on our professional experience, we know some number of factors or parameters may potentially contribute to AMR a fortiori in developing countries. Among them we can enumerate ineffective policies regarding public health system, medical, pharmaceutical, trade, nutritional, environmental and social safe. Our analysis aims to contribute to direct the reflexion toward integrated sectorial approaches to combat AMR.
When we look at the case of Ethiopia, for example, it is a common practice that people could purchase and use antimicrobial agents from any private pharmacies without professionals/physician prescription. The rules and regulations on the rational use of drugs are practiced very loosely in the ground.
I have read a number of articles about using essential oils and botanical to interfere with microbe growth on a variety or agricultural and medical products. Since, botanical sources also have the ability to mutant for survival and effect, botanical sources might be the 'cure'

This is true that AMR is now a day very tough challenge globally in combating against infectious diseases. Dr. Saha has correctly mentioned that the problems are not uniform globally and so it has to be customized regionally. If we see the factors behind AMR, these may vary in case of rural and urban areas respectively even in the same province of a country. Discovery of new medicine is necessary but this is not the ultimate solution to prevent AMR. We need to explore different factors of AMR through concerted efforts and coordinated research with the involvement of different stakeholders like, Policymakers, Pharmaceutical Industries, Pharmaceutical wholesalers, Pharmacists, Formal Health care providers including Doctors, Informal Health Care Providers (those who provide primary health care service mainly in rural settings) and general people.
