its subjective, so you have to decide the force and amplitude of movement, definitely it will help in adhesive caps.

In my clinical practice I have had better results with manual therapy doing inferior glides (or other directions into the patient's particular joint restrictions).  This is more specific than traction.  

I think mobilisation with distraction could be more beneficial than sustain traction with pulley. Pulley could be used for auto-mobilisation and maintenance of available ROM 

Are we talking about the painfull, the freezing or the frozen phase? Are we talking about idiopathic or traumatic adh. capsulitis?

I have no experience with the shoulder but with adhesive capsulitis of the ankle arthrodiastasis has shown promise.  I know this is prolonged controlled distraction for several weeks so don't know how you would transfer this to the shoulder but in the ankle it has worked in many patients.

By adhesive capsulitis I mean Idiopathic not traumatic.Clinically we cant tell with surety that patient is freezing or frozen.Although duration of symptoms,pain and range of motion restriction helps to diagnose.Correct me if you have different view on this.

Hi.We use both stretching and mobilization. The following motions are performed : at first: hot pack  and then mobilization of shoulder girdle in form of rotational movement by gravity upper limb and at last : traction and movement with pully for increasing of range of motion and daily assessment of range of movement and increasing of it.

The questions of Geert Jeuring are essential. To my experience it's not until the last phase of idiopathic capsulitis, that this type of phys. therapy is beneficial (when improving whatever you do or don't do).

Totally agree with G Jeuring and H Ako. The phase during which the GH joint should be mobilized is essential. During the thawing - last phase (increase of active mobilization and decrease of pain) of both post traumatic or idiopathic capsulitis any mobilization is indicated to recap mobility. In the first - freezing phase it makes little sense to mobilize the GH joint - for it takes its "own direction" and is painful. Imagine parabola- like progression.

Thanks Alexia for the answer.I agree late phase mobilization is indicated to increase range of motion but we do give grade I and II mobilization in painful stage.Most of the researches which see the efficacy of mobilization include patients in phase II and III of adhesive capsulitis .
