The criteria could be subdivided into two classes: costs for various types of services and outcomes. These two classes are competing in the sense that outcomes can be improved by increasing the cost. My goal is to develop an index for measuring the performance of various service units. I am interested in the literature where the focus is on the development of models and I am also looking for examples of such models. Thanks in advance for any suggestions.

I would strongly reccomend you to read the atatched article about the "Macbeth" Methodology or, if you prefer, go directly to http://www.m-macbeth.com/en/m-home.html . There, download te "Macbeth" manual, follow the very simple but very illustrative example of how to construct and explore a decision model and, try the demo vertion of the software. But take into consideration that "Macbeth" is a very sophisticated and advanced method to dealwith real MCDA.
