The theory of planned behaviour posits that the formation of intentions are comprised of beliefs/attitudes, normative beliefs/subjective norms and control beliefs/perceived control. In some models, other variables have been used to achieve higher accuracy or account for variance, for example descriptive norms.
The theory works best when you are able to establish specific measurable behavioural outcomes that result from behavioural intentions. What entrepreneurial behaviours are you attempting to measure? Also, you need to understand what conditions facilitate the formation of the corresponding entrepreneurial intentions. Once you know the conditions, you will be better able to define which conditions support beliefs/attitudes, normative beliefs, control beliefs and so on. The articles below will help you find survey items and provide you with other important details:

PS:
Also, if you have a moment, I would appreciate your participation in my graduate survey study about ethics/intentions and behaviour in the workplace. The following link will take you to the questionnaire which takes about 3-4 minutes. Note, the study I am conducting is based on the theory of planned behaviour.
