Hi, NFHS-3 (available at www.dhsprogram.com) have diabetes prevalence at the national level, but are self reported. Also WHO-SAGE data (available at http://www.who.int/healthinfo/sage/cohorts/en/index2.html) have self reported diabetes for six LMICs.

NHANES database in the United States may have the type of data you are looking for.  They also have OGTT data on a large cohort of the US population along with many variables such as age, family hx, household size etc... Most of the data is available for free on the CDC website to download. If you want additional data, you may have to write to them.  You also need SAS or SUDAN statistical packages to analyze them.
