why is there a severe immunological and inflammatory explosion in those affected by sarms covid-19? are these only genetic causes related to the immune response of il6 and macrophagus, or would other types of scenarios be related to some environmental element?
Short answer, we dont know.
In the initial growth phase of the infection prior to a major immune response Covid-19 can gain a good head start and produce a good number of clones. When the immune system does finally kick in to its second phase it can overreact with cytokine storms, mass activation of NK T-cells etc and lead to the friendly fire situation where our own tissues are damaged.

Mauro Luisetto, because we are all lovely individuals with a vast array of genetic variation. The paper does address it in a few points, firstly if Covid moves into the "second phase" depending on how we reacted to the first phase we can have the off target damage from the immune system to our cells. They also point out HLA differences is a likely factor too in how we respond immunologically
