There are many problems with drug infiltration when using i.p./i.v. injection, because tumor has ECM outside and less vascular inside.
For drugs that can directly function on tumor cells instead of other cells, why don't we directly inject the drug into the tumor site???
I think the main consideration here is, if the tumors are localized and resectable, then perhaps surgical resection would provide not only quicker tumor removal, but also a better outcome. If the tumors are not localized and some tumor populations occur beyond the main tumor margins, intratumoral injections would have questionable effects as there, again, might not be adequate vasculature to provide good delivery of the drugs utilized to all of the tumors being targeted. Some intratumoral injection protocols do exist in delivery of viral-mediated therapy as they have better ability to spread to neighboring cancer cells (e.g. Imlygic)
