Most patients with prostate cancer do well with hormone ablation therapy +/- external beam radiation (intensity modulated radiation therapy). Even those patients with bone alone mets do well with such a strategy + zometa. Therefore, an important question is if surgery should be limited only to those patients who are "young" and have high Gleason scores or have locally extensive tumors. For older patients excessive treatment for patients with milder disease also needs to be addressed. Complications from surgery or radiation include incontinence and other bowel, urinary or sexual problems.
