In oncological practice the assesment of mitosis is the usual way to establish a prognosis. A low mitotic index indicates a better prognosis.
However, if your objective is to find out the efficacy of a treatment, the apoptotic index is a reliable issue to asses the response.
We are unable to establish which is more important because the objectives of these mesurements are different:
Mitotic index speaks about "how malignant" is the tumor and apoptotic index shows how effective is the treatment.
Theoritically we can divide malignant tumours progression and behavior according to the difference between their malignant cells mitosis and apoptosis .
1. Tumors with regression and Excellent prognosis
If apoptosis is more than mitosis.
2. Tumors with stationary state and Good prognosis
If apoptosis is equal to mitosis.
3. Tumors with progression and Bad prognosis
If apoptosis is less than mitosis
Lymphocytic infiltration in malignant tissue is good example of tumor immunity which can increase apoptosis and decrease tumor progression. It is proven to be a good prognostic factor.
Also chemotheraputic drugs which reduce malignant cell division and decrease mitosis is widly used for ttt of leukemias in children with good prognostic results.
