Pregnant 21 year old had IA mucinous ovary cancer at week 14th of pregnancy and underwent to oophorectomy + omentectomy+ peritonial biopsy + appendectomy due to her desire to lead pregnancy to term and have baby.
Pregnancy follows with no complications up to now and on February 25th she'll be 39,1 weeks and it will performed a cesarian section.
Our question is about cancer approuch. Is better patient clinical follow or radical surgery at the same moment of cesarian section?
In the absence of prospective randomized trials , the management of ovarian cancer in pregnancy is challenging. CS is a good time to access the progress of the tumor. depending on staging you can offer her radical surgery or chemotherapy.alone NO tumor progress was found.

How it treats of a so young woman and her first pregnancy, I think our responsability is too great because we are defining not only her maternal future, but also her life. It is a too difficult situation

Dear Claudio,
This is a very hard decision. As you know the surgical treatment for ovary cancer is a cytoreduction. So, I think to talk with the oncology team, with the patient and her family pointing all aspects will help you in this hard decision.

Really, it is a too challenging case but our group is confident to have a good final meanly for that patient wich is a too Young woman and we think she deserves that.
In relation to the oncologist opinion we are working with one team of them but we are still not sure about which way to go
Anyway, how we have some time to surgery procedure we solved bringing the case to discuss in this forum as manner to take the best possible conduct
