What is the best statistical test to use in understanding the relations between ordinal variables, i.e. I have N 1-20 items, each item may be used once, twice or thrice. I want to test the relations between items and uses versus outcome of : no events, unexplained event, severe event and death.

From the question posed, it's not clear what the nature of the independent variable is. In addition, the description "unexplained event" does not explicitly refer to rank=2 if the other ranks are supposed to be "no event"=1, "severe event"=3 and "death"=4.
