The decisión of performed a OAGB whit only 150 cms biliopancreatic limb  it  was wrong. The total lengh of small bowell should be measured, and then you can calculate the necessary  common limb and the excluded bilipancreatic limb.

Whit only 150 cms excluded and a posible 350 cms of common limb, its logical the weigth regain and again comorbidities. In my experience you have two options: the most simple is transformed in a Roux-Y with a common limb between 150-180 cms, but is this case you add an alimentary limb uncertenly. The second option is more safer, but you need conserved the primitive OAGB anastomosis and performed two news anastomosis; rise up the common limb in order to maintain a new common limb of 150-200 cms ( its your decisión ) an a longer biliopancreatic limb of 300 cms.
