First I know that some diseases occur as cycles like malaria and the reason is time of RBCs rupture. but what make gout do the same in cyclic attack although it is inflammatory disease urate and macrophage are present all time so why the attacks are not continous like other inflammations ?

Gout results due to accumulation of uricacid crystals in joints.Attacks are common in nights after 1-2 hrs.of sleep.No excretion of uric acid in night ,hypoxia induced uric acid formation have been implicated in increased urates in night ,thereby leading to painful cyclical attacks.

diet with high uric acid production, dehydration and renal issues.
prophylaxis with allopurinol will help.
