Questionnaire (questionnaire): The questionnaire is a tool for collecting data on the subject of research. It is a form that contains a set of questions directed by the researcher to the individuals concerned in the research (the research sample). (The answer is written). The questionnaire is one of the most widely used tools in the field of humanities.

While the checklist - liste de controle is a tool to follow up and recall a topic through a series of examples related to the different aspects of the topic, to help analyze and evaluate these aspects.

The questionnaire and the checklist are both an evaluation tool, but the difference between them is

• the questionnaire assesses the future (if we apply a concept in the future).
• While the checklist is to assess the current reality.

Questionnaires can be self-administered or interviewer-administered; can measure single or multiple constructs; contain two parts (questions/items and a response set); and, can use positive and negative worded items.

Inventories/checklists are catalogues of different attributes, attitudes, and perceptions, and are used to examine certain characteristics of participants.

The major differences between questionnaires, scales, and inventories are the levels of precision, items and scoring, and nature of phenomenon to be measured.

Regarding the difference you wish to verify (as stated above), I do not think the construct of 'current and future' makes an absolute distinction between the two tools.

A questionnaire can be developed to elicit information on current reality or thoughts for the future. For instance, if your interest is health behavior, e.g. screening, in the questionnaire you can have questions on current behavior, past behavior or intended adoption of behavior.

An observation checklist would be useful to record current behavior as observed by the researcher or other person.
