In our unit (a tertiary care, referral center) a review over a 2 year period revealed the incidence of UVC extravasation to be 4.2%. The most common mode of diagnosis was ultrasound of abdomen with the following features.
Wow, I am overwhelmed by your findings. Now I can not tell the family that UVC is a safe procedure. But, I would like to know if ultrasound is done on infants without UVC will you see similar signals? What I am trying to say is the reality that if you strongly believe a procedure is going to have complications then you tend to exhaust all your attention to find them. I have no doubt you were not biased intentionally but would like to raise the possibility.
We have also seen injuries related to UVC in preterm newborns(especially). Hepatic echogenicities - we wondered if these could be related to repeated attempts at UVC insertion and the associated hematoma. We have also seen what looks like localised air collection on the surface of the liver (on routine X-rays) secondary to difficult UVC insertions. these observations have all been anecdotal so far and we are seeing similar observations from neonatal units in the region.
I am surprised with your results.! Which persons (nurses, interns, paeditricians or neonatologists) in your NICU staff are responsible for setting UVC? Do you have a control group (infants without UVC)?
