A 27 year-old married woman was admitted due to pain in lower abdomen for 7 days. USG revealed a multilobbed and multiloculated cyst in the left lower quadrant. During laparotomy, the parietal peritoneum was found thick. It was opened. A cystic lesion containing straw colored fluid was found. loculi were broken. Biopsy was taken. She had amenorrhoea for last one year.
Prabhath any tumour markers done. Ovarian tumor of Benign origin , Borderline malignant ovarian Tuomr could be posiblities. tumour markers and the biopsy report will help in the managements.in my experience we do get huge ovarian tumours,difficult one also is it is a Teriary center.,i have come across of such ovarian multiloculted cysts of ovary with thick peritoneum as Atypical presentation of Tuberculosis also

so prabhath my possible diagnosis from your history was correct.i have operated such cases with atypical presentations of Tuberculosis which might resemble like malignancy on imaging modes.thank you

Thank you prabhath.u were good in managing the case.great.i will tell one patient whom we operated for carcinoma of ovary Radical Hysterectomy with lymphnnode dissection ,omentectomy.no tubercles seen any where but histopathology report showed epithelial carcinoma of ovary with Tuberculous cocoon. told just to share you in developing countries Tuberculosis can present in various presentations.thank you god bless you.
Nice case. In such cases its important to evaluate the whole abdomen on imaging USG or CT whichever modality is being used. And a radiologist should evaluate the patient first without history and then in the light of history/clinicals of the patient. Many a times the diagnosis missed or not considered in differentials because of being too focused on one disease/organ/subspeciality
