Exercise continues to be the first line treatment for uncomplicated OA of the knee (AHCPR) for several reasons. We understand that joint motion stimulates a natural joint lubrication which is important and protective against joint erosion. When we exercise the muscles surrounding the knee we are improving the support system -both primary (ligamentous) and secondary system (muscular). Mechanical forces have been shown to be critical for mechanotransduction of not only bone/cartilage cells, but every cell type in the body (manuscript being published in ppmjournal.com) in September/October issue with more detail. Exercise also can help with weight management thereby reducing joint loads which can protect a fragile joint. Exercise is critical for development of prorioceptive or joint stabilization functions which are vital in protecting the joint during dynamic activities from further injury.
