The Cause & Effect, or Fishbone Diagram, was first used by Dr. Kaoru Ishikawa of the University of Tokyo in 1943 - hence its frequent reference as a "Ishikawa Diagram". This diagram is used to identify all of the contributing root causes likely to be causing a problem.

In quality management in the pharmaceutical industry and research it is often used to identify in advance any possible problems that can negatively influence the use of analytical chemistry in pharmaceutical projects. You collect a group of experts and do a moderated brainstorming session, drawing an Ishikawa diagramm as you discuss (there are some free programs like XMIND that are very useful for that). That can then by used almost 1:1 to generate a FME(C)A risk analysis table. Any identified risks above a certain threshold are mitigated before you even start the first measurements. That significantly improves the quality of the analytical results.
The trick is to get the right experts in one room at the same time without external pressures from other projects for a set amount of time - risk management is not a job for the hasty and/or inattentive.
Dear Rathore sir., on this question Mr. Willibald Stumptner has shared his view and highlighted the importance of Ishikawa Diagram (Fish bone Diagram). This Ishikawa diagram is one of the Lean laboratory tool used to reduce or eliminate waste (Please see the attachments for details Page No 188 -189) which are likely to occur in any event (in your case; Analyical Chemistry ) by various factors like 6M's viz; Methods, Measurements, Machine, Man, Material and its Mother nature, so create a diagram and list out all possible errors which could come across in analytical chemistry, per say to carry a test or detect a particular compound, so with this Ishikawa diagram one can trace out errors at initial stage itself and it will save your resources...!!! faster and efficient results one can get it.

Ishikawa diagram was popularized by Kaoru Ishikawa, who pioneered quality management processes in the Kawasaki shipyards, and in the process became one of the founding fathers of modern management.
It is considered one of the seven basic tools of quality control, also known as a fishbone diagram because of its shape.
Management people are using it Strategic management also.
I used ti use such diagram as a poster presentation in explaining many engineering cases to my students like this one:
