To my knowledge there is not any such first or second line treatment for malaria as such. However there is treatment guideline with respect to the type of parasite found to be present in malaria fever patient as well as according to the complication/severity status.
You may like to refer the recent treatmment guideline issued by NVBDCP in 2013
If i may, can also ask a favor for one more thing related to malaria. I found that, 60% of Malaria vulnerable population of Asia, is residing in India and that, 30% of malaria associated death are by Cerebral malaria ! is there any recent survey or report that gives a summary of the Malaria incidences, Mortality, Complications, etc., ?
hello, there is no first or second line. ACT i.e artesunate or related compounds in combination treatment with doxycycline, clindamycin, mefloquin or sul plus pyre is standard for india to cover chloroquin resistant falciparum species.
You may find lot of information by reaching the different institutes/centres of ICMR working on malaria. First you open Indian Council of Medical Research Website at www.icmr.nic.in/ then open Research profile of ICMR institutes, In list of permanent institutes/regional medical research centres you may find National Institute of Malaria Research, Delhi, Regional medical research centre, Jabalpur and Desert Medicine Research Centre, Jodhpur etc working on malaria. You may browse the institute's profile to learn more about the works done on malaria in India. I think this will help you and solve problem
The treatment of malaria in India depends on whether it is due to P. vivax or P .falciparum . If it is due to P. vivax , Chloroquine is the drug of choice . If it is due to uncomplicated P. falciparum , oral Artemisinin Combination Therapy ( ACT ) is used . In India , Artesunate - Sulfadoxine / Pyrimethamine is recommended . If it is severe malaria , IV Artesunate & Doxycycline / Clindamycin is used . It is essential that diagnostic tests to diagnose the type of malaria are made available . Only if tests reveal if it is vivax or falciparum , should the appropriate drug be used , This to prevent drug resistance to ACT . This policy has evolved because of the strategy in India to treat all cases of malaria with chloroquine as first line drug , which led to the emergence of P , faciparum as the major cause of malaria in various states leading to high mortality .
