A 72-year-old male, presented with cough and fever of 20-day duration. Clinical examination of the chest revealed bronchial breathing in the right middle zone. Chest x-ray showed a patchy opacity involving the right middle lobe. Total leukocyte count was 16600/µl. He was prescribed cefepime 2 gm twice daily intravenously for 14 days and azithromycin 500 mg once daily for 3 days but the condition failed to improve clinically or radiologically.
